Summarise equipment reliability: mean time between failures, failure rate per 1,000 hours, and uptime — save each summary on this device.
Try sample values — fills the form with the worked example below.

MTBF (hours)
–
Failures per 1,000 h
–
Uptime (%)
Equation: MTBF (h) = operating hours ÷ failures. Failure rate (per 1,000 h) = failures ÷ operating hours × 1,000. Uptime (%) = operating hours ÷ (operating hours + downtime) × 100.
Method: Reliability summary per ISO/IEC 17025:2017, clause 6.4 (equipment): maintenance and failure records are kept so the laboratory can show an item is fit for use, and spot an item that keeps failing.
Assumptions: operating hours are the hours the item actually ran; downtime is the time it was out of service for a fault or repair.
Limitations: MTBF describes the past only — a rising failure rate matters more than one good average, so compare this summary with the last one.
Rounding: MTBF to 1 decimal place; failure rate to 2 decimal places; uptime to 2 decimal places; intermediates unrounded.
This is a calculation aid, not a laboratory report — confirm against your accredited method before citing a value. Records are kept only in this browser on this device (localStorage); this is a device-only record aid, not a shared multi-user LIMS, and nothing is sent to any server.

Worked example: 2,000 operating hours, 4 failures, 40 h downtime: MTBF 500.0 h, failure rate 2.00 per 1,000 h, uptime 98.04%.
