Conformance (%)

Audit score
–
Audit grade
Equation: Conformance (%) = conformances ÷ checklist items × 100. Non-conformities = minor + major + critical. Audit score = conformance (%) − minor × 0.5 − major × 2 − critical × 5, floored at 0. Grade: Fail when any critical finding exists or the score is below 70; Pass when the score is 85 or above with no critical finding; otherwise Conditional pass.
Method: Internal audit scoring per ISO 19011:2018 (guidelines for auditing management systems) and the non-conformity grading used by GFSI-benchmarked schemes (FSSC 22000, BRCGS Food Safety). The deduction weights (0.5 / 2 / 5) and the grade bands are the tool’s stated rule, not a scheme requirement.
Assumptions: every checklist item is counted once, as a conformance or as one non-conformity of a single grade.
Limitations: the score ranks the audit outcome — it does not replace the auditor’s judgement on any single finding, and a critical finding fails the audit whatever the score is.
Rounding: conformance and score to 2 decimal places; intermediates unrounded.
This is a calculation aid, not a laboratory report — confirm against your accredited method before citing a value.

Worked example: 60 items: 54 conformances, 4 minor, 2 major, 0 critical: conformance 90.00%, 6 non-conformities, score 90 − 2 − 4 = 84.00 — Conditional pass.

A critical finding fails the audit on its own — fix it, re-audit the area, and record the corrective action. Saved on this device only. Tool 260 of 500 — GIFSQ
