Track corrective actions after a CCP deviation: closure rate, on-time rate and the average correction time against the target — HACCP Principle 5.

Try sample values — fills the form with the worked example below.

–
Closure rate (%)
–
On-time rate (%)
–
Time vs target (%)
Equation: Closure (%) = actions closed ÷ deviations × 100. On-time (%) = actions closed on time ÷ actions closed × 100 (0 when none are closed). Time vs target (%) = average correction time ÷ target time × 100.
Method: Corrective action per Codex CAC/RCP 1-1969 (Rev. 2020), HACCP Principle 5: when monitoring shows a deviation, take action to regain control, dispose of the affected product safely, and prevent recurrence. ISO 22000:2018 clause 8.5.4.4 and clause 10.2 require the same, with records kept.
Assumptions: each deviation has exactly one corrective action; "on time" means closed within the target time; the average is the mean of the closed actions only.
Limitations: the rates describe how the workflow is performing — they do not judge whether any single corrective action was the right one. Root-cause review is a separate step.
Rounding: percentages to 2 decimal places; intermediates unrounded.
This is a calculation aid, not a laboratory report — confirm against your accredited method before citing a value.
Worked example: 12 deviations, 10 actions closed, 8 on time, average 3.5 h against a 4 h target: closure 83.33%, on-time 80.00%, time vs target 87.50%.

Every deviation needs a closed corrective action — open actions are audit findings, not paperwork delays. Tool 246 of 500 — GIFSQ
