Aseptic Technique Checklist

Score an aseptic-technique observation: checklist items passed, items failed, and any critical failures that fail the assessment outright.

Try sample values — fills the form with the worked example below.

Score (%)
–
Items failed
–
Assessment
Equation: Score % = items passed ÷ total items × 100. Items failed = total − passed. Pass only when score ≥ 90 % and critical failures = 0.
Method: Aseptic-technique observation against the laboratory's own checklist (hand hygiene, disinfection, sterile field, handling of caps and pipettes, waste disposal), following the aseptic handling principles of FDA BAM and ISO 14698 cleanroom biocontamination control. The 90 % pass mark with zero critical failures is a laboratory policy threshold, not a standard value.
Assumptions: every checklist item was observed and marked yes or no; a critical failure is an item the laboratory has classed as critical in advance.
Limitations: a checklist score measures observed behaviour on one occasion, not competence over time. It does not detect contamination that leaves no visible trace, so it sits alongside environmental monitoring, not instead of it.
Rounding: score to 1 decimal place. Counts are whole numbers.
This is a calculation aid, not a laboratory report — confirm against your accredited method before citing a value.
Worked example: 20 checklist items, 18 passed, 0 critical failures. Score = 18 ÷ 20 × 100 = 90.0 %, with 2 items failed and no critical failure, so the assessment passes.

Enter values above to see results.

Any critical failure — such as touching a sterile surface with bare hands — fails the assessment whatever the score. Retrain and re-observe before the analyst works unsupervised. Tool 77 of 500 — GIFSQ
