Form – Training Effectiveness Evaluation
Effectiveness evaluation asks whether the training worked – not whether people enjoyed it. Test it, observe it, or measure it in the KPIs; ‘everyone signed the sheet’ is attendance, not effectiveness.
Fill this in as you go, not from memory at the end of the shift – records reconstructed later are where the errors creep in. Replace [bracketed] guidance with your site’s detail.
